Assess the situation
The site and severity of bleeding and the patient’s stability are central to the initial assessment. Critical-site or life-threatening bleeding requires urgent clinical care; a public website is not an emergency-management service.
Bring the relevant information together
The anticoagulant, recent intake, renal function, concomitant medicines and available tests help inform a clinician-led review. Different agents have different reversal considerations, which must be checked against current guidance and local availability.
Think beyond the immediate episode
After bleeding is controlled, reassessment includes the reason for anticoagulation and the balance of thrombotic and recurrent bleeding risk. Restarting treatment is an individual clinical decision. The ACC pathway addresses both bleeding management and subsequent treatment decisions.
The key idea
Severity, urgency and clinical context guide the pathway. The clinician determines the action.
