Understand the distinction

DOAC therapy generally does not use routine INR-guided dose adjustment in the way warfarin does. It still requires clinical review. Renal function, blood count and liver function can be relevant to safe prescribing and ongoing assessment.

Follow the patient over time

Reviews provide an opportunity to discuss adherence, new medicines, bleeding symptoms and changes in health. The appropriate review interval depends on individual circumstances, including frailty, kidney function and intercurrent illness.

Keep the purpose clear

Monitoring is more than ordering a test. It links a result or a change in health to a prescribing review and a documented follow-up plan. Use the applicable product information and local clinical guidance to determine what is needed.

The key idea

A medicine that does not require routine INR testing still requires an active follow-up plan.
