Different decisions within one condition
Atrial fibrillation management includes several clinical questions. Rate or rhythm management and prevention of thromboembolic stroke are related, but they are not the same decision.
Use the relevant framework
The Canadian Cardiovascular Society’s 2020 AF guideline describes the CHADS-65 approach to identify patients for oral anticoagulation. Risk assessment must be interpreted alongside the patient’s clinical history and the applicable current guidance.
Return to the whole patient
Bleeding-related factors, other indications, medicines and patient preferences belong in the discussion. Clinical reviews should revisit the plan as health and circumstances change. A score can structure a conversation; it does not replace an individual assessment.
The key idea
Use a recognised local framework and make the rationale understandable to the patient and the care team.
